 <description>&lt;p&gt;The Community Foundation Sonoma County provided the &lt;a href="/communitynetwork"&gt;Community Network&lt;/a&gt; with part of the seed money  needed to establish the &lt;a href="/journey"&gt;Journey Project&lt;/a&gt;. Community Foundation Sonoma County, with assets exceeding $130 million and more than 450 different funds established by individual donors, community groups and nonprofit organizations, has returned more than $116 million to the community since it was established in 1983.&lt;/p&gt;</description>

 <title>Exchange Bank</title>

 <description>&lt;p&gt; &lt;a href="https://www.exchangebank.com/"&gt;The Exchange Bank&lt;/a&gt; awarded the Community Network a grant in 2010 to replace the computer system we rely upon for all that we do.  This grant, awarded through the Exchange Bank Foundation, also funded purchase of a portable &lt;span class="caps"&gt;DVD &lt;/span&gt;player for our small group training efforts. Additionally, the Exchange Bank helped to fund a three-hour pilot training for administrators and staff from Residential Care Homes for the Elderly that we produced  in late October, 2010 titled Improving Care Through Life&amp;#8217;s End for People in Residential Care: Challenges and Opportunities.&lt;/p&gt;</description>

 <title>Jean Schulz Donor Advised Fund</title>

 <description>&lt;p&gt;In the mid-1990s, the Jean Schulz Donor Advised Fund granted the &lt;a href="/communitynetwork"&gt;Community Network&lt;/a&gt; money needed to purchase computer equipment used to launch and help grow the &lt;a href="/journey"&gt;Journey Project&lt;/a&gt;. In 2005 we were again generously awarded grant funds needed to upgrade our computer technology and incorporate the use of digital technology into our workshop program.  New technology is housed in the Environmental Center (home of the Journey Project Resource Center) and is shared for the benefit of members of the Sonoma County Conservation Council/Environmental Center. The Jean Schulz Donor Advised Fund is administered by the &lt;a href="/links/sonomacf"&gt;Community Foundation Sonoma County&lt;/a&gt;.&lt;/p&gt;</description>

 <title>Kaiser Permanente Santa Rosa</title>

 <description>&lt;p&gt;A grant award from &lt;a href="http://www.kaisersantarosa.org/"&gt;Kaiser Permanente (KP) Santa Rosa&lt;/a&gt; Community Benefit enabled the Community Network to work in concert with KP Health Education staff and area senior centers to produce four &amp;#8220;Wellness Through Life&amp;#8217;s End&amp;#8221; workshops in Santa Rosa and Sebastopol during 2007 (see calendar for more info.) The Kaiser grant award also helped with upgrading our website and building the &amp;#8220;Support Group Clearninghouse&amp;#8221; serving consumers and providers in the Greater Sonoma County region.           It is the mission of Kaiser Permanente to improve the health of KP members and the communities served by &lt;span class="caps"&gt;KP.&lt;/span&gt;&lt;/p&gt;</description>

 <title>Pacific Foundation for Medical Care</title>

 <description>&lt;p&gt;The Pacific Foundation for Medical Care granted the &lt;a href="/communitynetwork"&gt;Community Network&lt;/a&gt; a part of the seed money needed to start the &lt;a href="/journey"&gt;Journey Project&lt;/a&gt; in 1997. In 2011 &lt;span class="caps"&gt;PFMC &lt;/span&gt;granted us funding needed to help further our work educating physicians about end of life planning and care. The Pacific Foundation for Medical Care is a nonprofit organization dedicated to improving patient access to physicians. Through their affiliation with the California Foundation for Medical Care, they are part of one of the largest preferred provider organization (PPO) in California, with more than 35,000 physicians, hospitals, and other health care providers throughout the state.&lt;/p&gt;</description>

 <title>St. Joseph Health  System - Sonoma County</title>

 <description>&lt;p&gt;The &lt;a href="http://www.stjosephhealth.org/aboutus/default.htm"&gt;St. Joseph Health System Sonoma County&lt;/a&gt; Community Benefit Department provided seed money for the Journey Project Latino Caregiver Outreach &amp;amp; Training Program development during 2006. The grant enabled us to do work with community leaders ( including the Journey Project Latino Advisory Council) to develop a Plan for Cooperative Action.  This Plan will help guide and support the expansion, decentralization and integration of the Journey Project services into the existing Latino service delivery system.            It is interesting to note that the &lt;span class="caps"&gt;SJHS&lt;/span&gt;-SC, founded and operated by the Sisters of Orange, has been serving the Sonoma County region since New Year&amp;#8217;s Day 1950 when Memorial Hospital opened its doors.&lt;/p&gt;</description>

 <title>The California Endowment</title>

 <description>&lt;p&gt;Funding from &lt;a href="http://www.calendow.org/"&gt;The California Endowment&lt;/a&gt; assisted with development of the tools, resources and capacity to further develop our Wellness Through Life&amp;#8217;s End two-hour training program in Spanish for Latino caregivers. In Fall, 2008 Journey Project Latino Program Co-Directors presented the pilot training program &lt;em&gt;Bienestar A Traves del Final de la Vida, Encontrando Nuestro Camino&lt;/em&gt; to more than 30 Latino caregivers as a result. Work is underway, doing what is needed to continue this program created with 2008 seed funding from The California Endowment (TCE).&lt;/p&gt;</description>

 <title>The California HealthCare Foundation</title>

 <description>&lt;p&gt;&lt;a href="http://www.chcf.org/"&gt;The California HealthCare Foundation&lt;/a&gt;  provided a two-year grant (2009-2010) for the Community Network to develop and support the &lt;em&gt;Sonoma County &lt;span class="caps"&gt;POLST&lt;/span&gt; Implementation Program&lt;/em&gt;. This effort is helping to establish the Physician&amp;#8217;s Orders for Life Sustaining Treatment (POLST) as a recognized and widely-used tool in Sonoma County. &lt;span class="caps"&gt;POLST &lt;/span&gt;helps to ensure that patient treatment wishes at the end of life are honored and carries the force of law in CA effective January 1, 2009.  &lt;/p&gt;
