The California HealthCare Foundation provided a two-year grant (2009-2010) for the Community Network to develop and support the Sonoma County POLST Implementation Program. This effort is helping to establish the Physician’s Orders for Life Sustaining Treatment (POLST) as a recognized and widely-used tool in Sonoma County. POLST helps to ensure that patient treatment wishes at the end of life are honored and carries the force of law in CA effective January 1, 2009.
The California HealthCare Foundation is an independent philanthropy committed to improving the way health care is delivered and financed in California. By promoting innovations in care and broader access to information, our goal is to ensure that all Californians can get the care they need, when they need it, at a price they can afford. CHCF is currently giving special emphasis to improving end of life care.
