Sonoma County Health Action

Health Action envisions that, by the year 2020, Sonoma County will be the healthiest county in California to live, work, and play… a place where people thrive and achieve their life potential. This effort will take all of us working together to create a healthier community through collective action. Numerous programs are underway to help realize that vision. Committees are working ongoing to improve quality of life and health care across the continuum of care especially for those living with serious life-limiting illness nearing the end of l ife.
